Nationwide Children's Hospital, a 354-bed facility in Columbus, Ohio is among the most comprehensive, free-standing pediatric academic medical centers in the nation. The hospice program was established in 1995 and has served hundreds of children, select adults and their families. In late 2007, the program expanded to offer Home-based Palliative Care.

Nationwide Children's is seeking a dedicated and enthusiastic Patient Care Coordinator responsible for coordinating care and services provided to Hospice and Palliative Care patients and families. The candidate will supervise the day to day operations and the inderdisciplinary team. This is a full time (M-F day shift) position with some on-call responsibility.


Qualifications:

•A graduate of an accredited school of nursing. Bachelor’s degree in nursing preferred.
•Hold a current, valid Ohio Nursing License.
•Two to three years home health or hospice experience preferred/pediatric experience and supervision a plus.
•Valid Ohio driver’s license and proof of insurance.
•Willingness to accept new responsibilities commensurate with the growth of the agency.
•Strong communication skills needed to promote and maintain agency standards.
•Strong ability to problem solve with families, staff and other community agencies in order to provide quality patient care.
•Knowledge of Medicare/Medicaid Conditions of Participation and Waiver Regulations preferred.
•Candidates must have excellent critical thinking, communication, teaching and customer service skills.
