In collaboration with all appropriate members of the heath care team, the Advanced Practice Nurse will develop and implement clinical plans of care for adult patients with chronic and terminal illnesses. This position will perform a wide range of high-level activities, which include: assessment, history and physical; recommendations on aggressive symptom management; develop plans to meet patient and family physical and psychosocial needs; monitor patient progress; update the team; consult around discharge planning; and patient/family teaching.
Requirements • Current license to practice as a Registered Nurse/ Advanced Practice RN in the District of Columbia • Four years of nursing experience, preferably to include at least one year of palliative care, hospice or pain management experience • Successful completion of the Nurse Career Battery • Certification in specialty preferred
